Nursing Home Transition & Diversion Waiver (NHTD)ElderChoice | Syracuse, NYElderChoice is an approved Medicaid provider of Nursing Home Transition and Diversion Waiver services for Central New York including: the Syracuse, Oneida, Cortland, Ithaca, Rome & Utica areas. What is the Nursing Home Transition & Diversion (NHTD) Waiver? The NHTD program is a New York State Medicaid Waiver program that offers individuals with disabilities and seniors who are eligible for nursing home placement to receive services in their home and community. This option allows for a greater quality of care in the least restrictive setting possible. ElderChoice is approved to provide the following services through this program: Service Coordination Assists the individual in applying for the TBI Waiver and supports the participant in monitoring all services being provided in order to ensure quality, continuity and person centeredness. Home and Community Support Staff (HCSS) HCSS is a service designed to support and assist the individual with activities of daily living, both in their home and community. All HCSS are certified personal care aides. Respite This service is designed to provide natural supports that tend to a loved one’s daily needs with a “relief” period. This service can only be utilized in 24 hour periods and is congruent with the services offered by the HCSS/PCA staff. Eligibility & Referrals
The NHTD Waiver Program is a choice-based model; however, referrals must first be made to the Department of Health’s Regional Resource Development Center. After which the participant is free to interview and choose ElderChoice as their service provider, if they so desire. |
