Traumatic Brain Injury (TBI)ElderChoice | Syracuse, NYElderChoice is an approved Medicaid provider of Traumatic Brain Injury services for Central New York including: the Syracuse, Oneida, Cortland, Ithaca, Rome & Utica areas. What is a Traumatic Brain Injury (TBI)? Traumatic Brain Injury (TBI) does not discriminate. It knows no socioeconomic barriers, cultural or religious affiliations. A brain injury can happen to anyone, at any age, at any time. Any accident or illness that affects the brain can render a person less able to live and function in their home and community. This is not only devastating for the individual, but their families, friends and loved ones as well. Our team of professionals is specially trained and experienced in supporting individuals with brain injuries as they regain their independence and reach their ultimate life goals. ElderChoice provides the following TBI Waiver services: Service Coordination Assists the individual in applying for the TBI Waiver and supports the participant in monitoring all services being provided in order to ensure quality, continuity and person centeredness. Independent Living Skills Training (ILST) An individualized service, provided for the person in their home and/or community, which focuses on re-teaching life skills. Home and Community Support Staff (HCSS) HCSS is a service designed to support and assist the individual with activities of daily living, both in their home and community. All HCSS are certified personal care aides. Community Integration Counseling (CIC) CIC provides counseling specific to brain injury related issues, such as the person’s changing role in the family and the individual’s view of him/herself. Structured Day Program (SDP) Structured Day Program is a group service, which provides individuals the opportunity to socialize, re-integrate into their community and develop additional leisure and recreational skills in a supported environment. The TBI Waiver Program is a choice-based model; however, referrals must first be made to the Department of Health’s Regional Resource Development Center. After which the participant is free to interview and choose ElderChoice as their service provider, if they so desire. |
